The Allergies section sets a is used to add the patient's allergies similarly to the Allergies Clinical tab in the Patients ability. Allergies which have already been added for the patient will display within the Allergies section, where allergies can also be verified or added.

Section Header:

drug, seasonal, and food allergies to the patient record, similar to how they are added in the Clinical ability. This section will automatically pull in any existing allergies from the Clinical ability, as well as allergies added on past forms. If a new allergy is added to this section on the form, that allergy will then also show in the Allergies widget in Clinical.

The contents of this section are as follows:

  • Active/All: Active option shows only allergies marked as "Active" / All shows entire history of allergies, both active and inactive.

  • Allergies Verified: Select the Allergies Verified checkbox if the Allergies have been verified with the patient.

  • No Active Allergies: Select the No Active Allergies checkbox if the patient has no active allergies.

  • Allergies: Click the Allergies link hyperlink to add new allergies for the patient. In the resulting popover, search for the a specific allergy or click the Show All button to type three spaces and press Enter to load and select from all allergies within the MacPractice database.

  • Add Allergies List for Reconciliation: The Allergies section can also be used to add an allergies list from a referrer for reconciliation. Within the Reconcile menu, select Add New List for Reconciliation to add a new list of allergies to reconcile. In the resulting sheet, select the source of the allergy list.

Referral:
Existing referral for this patient: Click the Referral radio button, then select the existing referral from the menu if the patient's referral has already been added to the Patient > Referrals tab.
Create a new referral for this patient with an existing referrer: Click the Referral radio button, then search existing referrers as added to References > Referrals.
Create a new referrer and a new referral for this patient: Click the Referral radio button and add the Referrer First and Last name to the fields to create a new Referral reference and add the referral to this patient.

Patient:

Add a on-Referral List: Click the Patient radio button if you are just adding a list per the patient.

Click the Next button to continue adding an Allergies list for reconciliation. Within the resulting Allergies sheet, click Allergies to add a new allergy from the referral. In the resulting popover, search for the allergy or click the Show All button to select from all allergies within the MacPractice database. Repeat this process for each allergy to add to the list. Once finished, click the Next button.

Within the resulting sheet, select any allergies to retain on the patient record, then click the Next button. Allergies which are not selected will be removed from the patient Allergy list.

Confirm the displayed allergies should be kept on the patient record with the Next button, or click the Previous button to return to list of all possible allergies to retain.

  • Specific Allergy link popover:

    • Severity: Popup to specify severity of reaction from a set list of options

    • Identified Date: Date allergy was discovered

    • Onset Date: Date allergy started showing symptoms

    • Allergy Type: Popup to specify type of allergy from a set list of options

    • "Active" checkbox: Specifies allergies that the patient currently deals with

    • Reaction: Used to search and select the type of allergic reaction from an official database

    • Notes: Freeform text for any additional information involving the allergy

    • "Delete" button: Deletes selected allergy from patient record (Form and Clinical Summary)

    • “Done” button: Commits any added changes to the Allergies section
