Invalid Claim Frequency Code:

Rejection Message:

  • CLAIM FREQUENCY TYPE CODE IS INVALID. EXPECTED VALUE IS "1" (ORIGINAL CLAIM)

This rejection is stating that the payer does not accept corrected/voided claims. When a claim is sent as either corrected or voided it is assigned a 'Claim Frequency Type Code' of '7' or '8' respectively. This rejection is stating that the payer only accepts a 'Claim Frequency Type Code' of '1' which is essentially a 'new claim'. Usually, when this rejection comes up a rebuild of the claim will strip out the '7' or '8' and send it with the requested '1' qualifier.

Note: Medicare DOES NOT accept corrected claims. This rejection shows up most often when attempting to send Medicare a corrected claim. Please contact Medicare for proper procedure of submitting corrected claim information; should that be required.
