Patients By Insurance Carrier Report

The Patients By Insurance Carrier Report lists patients with a specific Insurance Company listed on their Account.

Filter Options

  • Provider: Filters the results by the Provider set in the Patient Ability under the Patient tab.

  • Offices: Filters the results by the Office set in the Patient Ability under the Patient tab.

  • Insurances: Filters the results by the Insurance Companies listed.

  • Plans: Filters the results by the Insurance Plan associated to the Insurance Company and patient.

  • Employers: Filters the results by the Employer associated to the Patient.

  • Start Date/End Date: Sets a date range by which to filter the report results. The Date Selector radio buttons allow you to choose what type of date to filter the results by. You can choose to filter by the Appointment Date, the Production Date, or the Insurance Start Date. You can also choose to Ignore the Date Range, in which case the report will simply pull all relevant records based on other filter criteria.

  • Special Ins. Only: Filters results to only those which have had the "Special Insurance" checkbox marked on the Patient tab, within the Insurance sub-tab.

  • Include Inactive Patients: This checkbox determines whether archived patients are pulled in this report.

  • Carrier Name: The name of the Insurance Company.

  • Address: The Address of the Insurance Company.

  • Phone: The Phone number of the Insurance Company.

  • # of Patients: The number of patients for this Insurance Company result.

First Expansion

 

  • Plan Name: The Plan Name for the Insurance Company result.

  • Address: The address for the patient.

  • Phone: The phone number for the patient.

  • # of Patients: The number of patients for this Plan name within this Insurance Company result.

Second Expansion 

 

  • Patient #: The patient number for the patient with the Appointment Type within the Insurance Company result.

  • Patient Last, First (Age, Sex): The first and last name, age, and sex of the patient with the Appointment Type within the Insurance Company result. Employer Name: The name of the employer on the patient account.

  • Employer Name: The name of the patient's Employer marked on the Insurance Company information in Patients > Primary/Secondary.

  • Subscriber Num: The Subscriber Number of the patient with the Appointment Type within the Insurance Company result.

  • Group #: The Group number for the patient associated to the Insurance Company result.

  • Address: The address for the patient.

  • Phone: The phone number for the patient.

  • Plan Name: The Plan Name for the Insurance Company result.

  • Office/Provider: The Office ID/Provider User ID associated to the Patient.
