Overview
Percentage of patients 65 years of age and older who have ever received a pneumococcal vaccine.

A patient will be in the denominator when they have all of the following- 

  • An age of 65+ years

  • A visit during your reporting period

A patient will be in the numerator when they have any of the following- 

  • Pneumococcal Vaccine Immunization

OR

  • Pneumococcal Vaccine Administered procedure 

OR

  • Pneumococcal Vaccine Administered or History of Pneumococcal Vaccine snomed code (to indicate a past vaccination) with a Start Date and End Date
