OverviewThe Childhood Immunization Status report measures the percentage of children 2 years of age who had four diphtheria, tetanus and acellular pertussis (DTaP); three polio (IPV), one measles, mumps and rubella (MMR); three H influenza type B (HiB); three hepatitis B (Hep B); one chicken pox (VZV); four pneumococcal conjugate (PCV); one hepatitis A (Hep A); two or three rotavirus (RV); and two influenza (flu) vaccines by their second birthday.

  • Denominator: Children who turn two years of age during the measurement period and who have a visit during the measurement period.

  • Numerator: Children who have evidence showing they received recommended vaccines, had documented history of the illness, had a seropositive test result, or had an allergic reaction to the vaccine by their second birthday.

Troubleshooting

  • The Patient needs to have a SNOMED, CPT, or CVX of the DTap Vaccine. This must be entered with all four stages on separate days and while the child is between 42 days and 2 Years old.

    • Or they must be diagnosed with Anaphylactic Reaction to DTap Vaccine, Encephalopathy, or resolved Encephalopathy.

  • The Patient needs to have a SNOMED, CPT, or CVX of Inactivated Polio Vaccine. This must be entered with all 3 stages on separate days and while the child is between 42 days and 2 Years old. 

    • Or they must be diagnosed with Anaphylactic reaction to Inactivated Polio, Streptomycin, Polymyxin, or Neomycin.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Measles, Mumps and Rubella Vaccine before they are 2 years old.

    • Or they must be actively diagnosed with Disorders of the Immune System, HIV, Malignant Neoplasm of Lymphatic Tissue, or Anaphylactic Reaction to Neomycin.

    • Or they must have active, resolved, or test Seropositive for the Antigen Test for Measles, Mumps, and Rubella.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Hemophilus Influenza B Vaccine. This must be entered with all 3 stages on separate days and while the child is between 42 days and 2 Years old.

    • Or the child must be diagnosed with an Active Anaphylactic Reaction to Hemophilus Influenza B (HiB) Vaccine.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Hepatitis B Vaccine. This must be entered with all 3 stages on separate days and before the child is 2 Years old.

    • Or they must be diagnosed with Anaphylactic Reaction to Hepatitis B Vaccine or Common Baker's Yeast.

    • Or have Active, Inactive, Or Test Seropositive for Hepatits B.

  • The Patient needs to have a SNOMED, CPT, or CVX of Varicella Zoster Vaccine before the child's second birthday.

    • Or be diagnosed with Active Disorders of the Immune System, HIV, Malignant Neoplasm of Lymphatic Tissue, or Anaphylactic Reaction to Neomycin.

    • Or have Active, Inactive, Or Test Seropositive for Varicella Zoster.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Pneumococcal Conjugate Vaccine. This must be entered with all four stages on separate days and while the child is between 42 days and 2 Years old.

    • Or they must have an Active Anaphylactic Reaction to Pneumococcal Conjugate Vaccine.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Hepatitis A Vaccine before the Patient is 2 years old.

    • Or they must be diagnosed with Anaphylactic Reaction to Hepatitis A Vaccine.Or have Active, Inactive, Or Test Seropositive for Hepatits A.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Rotavirus Vaccine (Two Dose Schedule.) This must be entered with all 2 on separate days and when the child is between 42 days and 2 Years old.

    • Or SNOMED, CPT, or CVX of Rotavirus Vaccine (Two Dose Schedule.) This must be entered with both on separate days. With an additional first dose from a 3 Dose Schedule.

    • Or the Patient needs to have a SNOMED, CPT, or CVX of the Rotavirus Vaccine (Three Dose Schedule.) This must be entered with all 3 on separate days and when the child is between 42 days and 2 Years old.

    • Or the child must have and Active Anaphylactic Reaction to the Rotavirus Vaccine.

  • The Patient needs to have a SNOMED, CPT, or CVX of the Influenza Vaccine. This must be entered with both stages on separate days and while the child is between 180 days and 2 Years old.

    • Or the child must be diagnosed with Active Anaphylactic Reaction to Influenza, Disorders or the Immune System, HIV, Malignant Neoplasm of Lymphatic Tissue, or Anaphylactic Reaction to Neomycin.
