Depression Remission at Twelve Months Report

Overview
The Depression Remission at Twelve Months Report tracks, the percentage of adolescent patients 12 to 17 years of age and adult patients 18 years of age or older with a diagnosis of major depression or dysthymia who reached remission 12 months (+/- 60 days) after that event.

Patients are split into two stratum:

Stratification 1: Ages 12 to 17
Stratification 2: Ages 18 and older

A patient will be in the denominator when they have all of the following:

• An age either 12 to 17 years of age (Stratum 1) or 18 years or older (Stratum 2)

• A visit anywhere from 13 months to 1 month prior to the Start Date of your reporting period

• A Major Depression Including Remission or Dysthymia diagnosis with an Onset Date matching the Procedure Date of their office visit

• A PHQ-9 score LNC code entered or imported to a Laboratory order with an Received Date matching the Procedure Date of their office visit and a Value over 9

A Patient will be excluded from the denominator when:

• They have a date, time and cause of death registered in the Patients Ability > Patient Tab > Alerts Sub-Tab.

• They have received hospice or palliative care services during the reporting period.

• They were permanent nursing home residents as of the reporting period.

• If the patient has a diagnosis of bipolar disorder, personality disorder, schizophrenia, psychotic disorder, or pervasive development disorder prior to their visit.

A patient will be in the numerator when they have all of the following- 

• A PHQ-9 score LNC code entered or imported to a Laboratory order with an Ordered Date within 11 to 13 months after the Procedure Date of their office visit and a Value under 5.
