Referral Data Pull Fields

The Referral Data table contains the same pull fields that are under the Primary, Tertiary, and Secondary Referrals pull fields section. It also contains referral pull fields located under the Incident pull fields section. The Referrals table can pull any patient referral upon creation of a note, rather than specifically a primary, secondary, or tertiary referral. 

  • Last Name: Referral's last name

  • First Name: Referral's first name

  • Title: Referral's title

  • Suffix: Referral's suffix

  • Nickname: Referral's nickname

  • Street Address: Referral's street address

  • Suite: Referral's suite; will only display the field's data

  • City: Referral's city

  • State: Referral's state

  • Zip Code: Referral's zip code

  • Email: Referral's email address

  • Taxonomy Code: Referral's taxonomy code; This will only display the alpha-numeric code and not the description.

  • Home Phone Number: Referral's phone number associated with the Home phone pop-up button as entered in one of the phone fields

  • Work Phone Number: Referral's phone number associated with the Work phone pop-up button as entered in one of the phone fields

  • Cell Phone Number: Referral's phone number associated with the Cell phone pop-up button as entered in one of the phone 
    fields

  • Fax Number: Referral's phone number associated with the Fax phone pop-up button as entered in one of the phone fields

  • Office: Referral's office association, as entered in the Office/Company field of the referral record.

  • Department: Referral's department

  • Position Title: Referral's title/position

The following pull fields' information can be found under the Provider IDs tab of the Referral Reference.

  • Self Pay: Referral's self pay number

  • Commercial: Referral's commercial number

  • Central Cert: Referral's central cert. number

  • Title V: Referral's title V number as enterer

  • Workers Compensation: Referral's workers compensation number

  • BCBS: Referral's BCBS number

  • Self Admin: Referral's Self Admin number

  • Vets Admin: Referral's Vets Admin. number

  • Medicare: Referral's Medicare Part B number

  • Champus: Referral's Champus number

  • PPO: Referral's PPO number

  • Medicaid: Referral's Medicaid number

  • HMO: Referral's HMO number

  • Managed Care: Referral's Managed Care number

  • Other: Referral's other non-federal. number as entered in the Other Non-Federal field

  • Other Federal: Referral's other federal number as entered in the Other Federal field

  • Federal Employee: Referral's Federal Employee number

  • Blue Cross: Referral's Blue Cross number

  • State License: Referral's State License number

  • National Provider ID: Referral's National Provider ID number

  • SSN/TID: Referral's SSN/TID number

  • Disability: Referral's Disability number

  • Expiration Date: This will pull the Expiration Date for the referral. The expiration date is calculated by adding the Months selection to the Date column. For the Months column to be displayed, go to MacPractice Preferences, then select Localization in the sidebar. Make sure the User Referrals Expiration Date is checked. If this is not enabled, the referral's expiration will be set to a term of on year.

  • Expiration Months: This will pull the selection chosen in the Months column on the Referrals tab of the Patient tab of the Patient ability. For this column to be displayed, go to MacPractice Preferences, then select Localization in the sidebar. Make sure the User Referrals Expiration Date is checked. If this is not enabled, 12 will be pulled by default.

  • Date: Date entered for the referral, under the Referral tab.
