Incident Pull Fields

The Incident pull fields all pull from the Incident node of the patient's ledger and can be edited there. If the patient has multiple incidents the most recent one is pulled, based on Incident Date. If a specific field is not noted, the name of the pull field is the same as the name of the field on the incident.

  • Incident Date: Date of the incident

  • Incident Name: Name of the incident

  • Incident Type: Type of incident

The Referral pull fields will list the information for the Referral listed on the Incident tab. All information, unless otherwise noted, can be changed in the References Ability > Referrers.

  • Referral Last Name: Referral's last name

  • Referral First Name: Referral's first name

  • Referral Title: Referral's title

  • Referral Nickname: Referral's nickname

  • Referral Street Address: Referral's street address

  • Referral Suite: Referral's suite; only displays the data entered in the field

  • Referral City: Referral's city

  • Referral State: Referral's state

  • Referral Zip Code: Referral's zip code

  • Referral Email: Referral's email address

  • Referral Taxonomy Code: Referral's taxonomy code; will only display the alpha-numeric code and not the description

  • Referral Type: Referral's Type

  • Referral Type Emergency: True or false pull field that will display T if the referral type field of the selected referral is set to Emergency Room and F if this field is set to anything else, or when no referral type is selected.

  • Referral Type Patient: True or false pull field that will display T if the referral type field of the selected referral is set to Patient and F if this field is set to anything else, or when no referral type is selected.

  • Referral Type Phone Book: True or false pull field that will display T if the referral type field of the selected referral is set to Phone Book and F if this field is set to anything else, or when no referral type is selected.

  • Referral Type Provider: True or false pull field that will display T if the referral type field of the selected referral is set to Provider and F if this field is set to anything else, or when no referral type is selected.

  • Referral Phone 1 Type: First Phone Number's pop-up field in the referral's reference. It will display a number representing the selection from 0-3 (if Home is selected from the pop-up menu, 0 will be displayed in the note. If Fax is selected a 3 will be displayed in the note).

  • Referral Phone 1: First phone number associated with the referral

  • Referral Phone 2 Type: Second Phone Number's pop-up field in the referral's reference. It will display a number representing the selection from 0-3 (if Home is selected from the pop-up menu, 0 will be displayed in the note. If Fax is selected a 3 will be displayed in the note).

  • Referral Phone 2: Second phone number associated with the referral

  • Referral Phone 3 Type: Third Phone Number's pop-up field in the referral's reference. It will display a number representing the selection from 0-3 (if Home is selected from the pop-up menu, 0 will be displayed in the note. If Fax is selected a 3 will be displayed in the note).

  • Referral Phone 3: Third phone number associated with the referral

  • Referral Date: Date entered for the referral, under the Referral tab (found under the Patient tab).

  • Referral Is Person: If the Is Person checkbox in the referral's reference is checked, 1 will be pulled into the note and when the box is unchecked 0 will be pulled into the note

    The following pull fields' information can be found under the Provider IDs tab of the Referral Reference:

  • Referral Self Pay: Referral's self pay number

  • Referral Commercial: Referral's commercial number

  • Referral Central Cert: Referral's central cert. number

  • Referral Title V: Referral's title V number

  • Referral Workers Compensation: Referral's worker's compensation number

  • Referral BC/BS: Referral's BCBS number

  • Referral Self Administration: Referral's Self Admin number

  • Referral Vets Administration: Referral's Vets Admin. number

  • Referral Medicare: Referral's Medicare Part B number

  • Referral CHAMPUS: Referral's Champus number

  • Referral PPO: Referral's PPO number

  • Referral Medicaid: Referral's Medicaid number

  • Referral HMO: Referral's HMO number

  • Referral Managed Care: Referral's Managed Care number

  • Referral Other: Referral's other non-federal. number as entered in the Other Non-Federal field

  • Referral Other Federal: Referral's other federal number as entered in the Other Federal field

  • Referral Federal Employee: Referral's Federal Employee number

  • Referral Blue Cross: Referral's Blue Cross number

The Facility fields all come from the Facility selected under the Incident node. The information can be edited in the References Ability > Facility.

  • Facility Name: Name of the facility

  • Facility Address: Street address of the facility.

  • Facility Suite: Facility's suite; only displays data entered in the field

  • Facility City: Facility's city

  • Facility State: Facility's state

  • Facility Zip Code: Facility's zip code

  • Facility Phone: Facility's phone number

  • Facility Code 1: Facility's tax ID

  • Facility Code 2: Facility's secondary identifier, as entered in the Secondary Facility ID field.

  • Facility NPI: Facility's NPI

  • Admission Source Code: Code for Admission Source entered on the Incident > Facility tab

  • Admission Type Code: Code for Admission Type entered in the Incident > Facility tab

  • Condition Related To: Description of the item selected in the Condition Related To pop-up button on the Incident > Reason for Treatment tab (for example, Employment, Auto Accident, Pregnancy, etc.)

  • Accident Date: Date entered in the Accident Date field on the Incident > Reason for Treatment tab

  • Accident State: Abbreviation of the state entered in the Accident State field on the Incident > Reason for Treatment tab (for example, Arizona will pull AZ, Nebraska will pull NE, etc.)

  • Occupational Illness: Yes/No pull field that will display No in the note if the Occupational Illness/Injury checkbox is unchecked and Yes if it is checked.

  • Prosthesis: Yes/No pull field that will display No in the note if the Prosthesis checkbox is unchecked and Yes if it is checked.

Unless otherwise noted, this information pulls from the attorney listed in the Attorney tab of the incident. It can be changed in the References Ability > Attorneys.

  • Attorney Address: Street address of the attorney

  • Attorney City: Attorney's city

  • Attorney Deposition Date: Date entered in the Deposition Date field on the Attorney tab of the incident

  • Attorney Email: Attorney's email address

  • Attorney Fax: Attorney's fax number

  • Attorney Final Settlement Date: Date entered in the Final Settlement field on the Attorney tab

  • Attorney Name: First and last name of the attorney

  • Attorney Notes: Text as entered in the Notes field on the Attorney tab of the incident

  • Attorney Notice Filing: Yes/no pull field that will display No in the note if the Notice of Filing box on the Attorney tab of the incident is unchecked, and Yes if it is checked.

  • Attorney Phone: Attorney's phone number

  • Attorney State: Attorney's state

  • Attorney Suite: Attorney's suite; only displays the data entered in the field

  • Attorney Trial Date: Date entered in the Hearing/Trial Date field on the Attorney tab of the incident

  • Attorney Zip code: Attorney's zip code

The Military fields are not available in MacPractice DDS. They are found under the Military tab in MacPractice MD, DC and 2020.

  • Effective Date: Effective Date field

  • Termination Date: Termination Date field

  • Military Date: Date field

  • Military Branch: Displays a number 0-7 representing the selection in the Branch pop-up menu. For example, if None is selected, 0 will be printed to the note. If CHAMP/VA is selected, 7 will pull.

  • Military Branch Army: True or false pull field that will display T if the Branch field is set to Army and F if this field is set to anything else, or none.

  • Military Branch Air Force: True or false pull field that will display T if the Branch field is set to Air force and F if this field is set to anything else, or none.

  • Military Branch Marines: True or false pull field that will display T if the Branch field is set to Marines and F if this field is set to anything else, or none.

  • Military Branch Navy: True or false pull field that will display T if the Branch field is set to Navy and F if this field is set to anything else, or none.

  • Military Branch Coast Guard: True or false pull field that will display T if the Branch field of the Military tab is set to Coast Guard and F if this field is set to anything else, or none.

  • Military Branch Public Health: True or false pull field that will display T if the Branch field of the Military tab is set to Public Health and F if this field is set to anything else, or none.

  • Military Branch CHAMP/VA: True or false pull field that will display T if the Branch field of the Military tab is set to CHAMP/VA and F if this field is set to anything else, or none.

  • Military Status: Displays a number 0-3 representing the selection in the Status pop-up menu under the Military tab. For example, if None is selected, 0 will be printed to the note. If Both is selected, 3 will pull.

  • Military Eligibility: Displays a number 0-5 representing the selection in the Eligibility pop-up menu under the Military tab. For example, if None is selected, 0 will be printed to the note. If Signed Statement is selected, 5 will pull.

The following fields can be found under the Claims tab in MacPractice MD, DC and 2020. They will be visible under the CMS Claims tab in MacPractice DDS, if "Display both CMS and ADA Claim view" is enabled in the Preferences > Ledger > Incident tab.

  • First Symptom Date: Date entered in the First Symptom field on the Claims > Symptom Dates tab. This field is not available in MacPractice DDS.

  • First Symptom Date Type: First Symptom Date Type popup in the Claims >Symptom Dates tab. This field is not available in MacPractice DDS. It will display a number representing the selected item from 0-3. If None is selected from the pop-up menu, a 0 will be displayed in the note. If Last Menstrual Period is selected a 3 will be displayed in the note.

  • First Symptom Text: If the Condition Related To pop-up button on the Incident tab of the incident is set to Auto Accident, this pull field will display Accident in the note. If the Condition Related To pop-up field on the Incident tab of the incident is set to anything other than Auto Accident and the First Symptom pop-up button on the Claims tab of the incident is set to either None or Routine Services, the pull field will be left blank. If Condition Related To is set to anything other than Auto Accident and the First Symptom pop-up field on the Claims tab of the incident is set to Illness or Last Menstrual Period, the pull field will pull those selections in and the note will display either Illness or LMP. This field is not available in MacPractice DDS.

  • Has Similar Symptom Date: This is currently under development.

  • Similar Symptom Date: Date entered in the Other Date field on the Claims > Symptom Dates tab. This field is not available in MacPractice DDS.

  • First Consultation Date: Date entered in the First Consultation field on the Claims > Symptom Dates tab. This field is not available in MacPractice DDS.

  • Partial Disability Begin Date: Date entered in the Begin Date field in the Disability tab > Partial Disability area

  • Partial Disability End Date: Date entered in the End Date field in the Disability tab > Partial Disability area

  • Return To Work Date: Date entered in the Return To Work Date field in the Disability tab > Partial Disability area

  • Total Disability Begin Date: Date entered in the Begin Date field in the Disability tab > Total Disability area

  • Total Disability End Date: Date entered in the End Date field in the Disability tab > Total Disability area

  • Box 10B Other Claim ID Qualifier: Data entered in the Box 10B Other Claim ID Qualifier field on the Claims > Other Claim Data tab

  • Box 10B Other Claim ID: Data entered in the Box 10B Other Claim ID field on the Claims > Other Claim Data tab

  • Box 10D Claim Codes: Data entered in the Box 11b Qualifier field on the Claims > Other Claim Data tab

  • Box 23B Payment Source Code: Data entered in the Box 23B Payment Source Code field on the Claims > Other Claim Data tab

  • Box 25C Locator Code: Data entered in the Box 25C Locator Code field on the Claims > Other Claim Data tab

The following fields can be found under the Incident tab in MacPractice DDS. They will be visible under the ADA Claims tab in MacPractice MD, DC, and 20/20, if "Display both CMS and ADA Claim view" is enabled in the Preferences > Ledger > Incident tab.

  • Initial Placement: True or false pull field that will display 1 if the Initial Placement checkbox is checked on the Incident tab and 0 if this box is unchecked.

  • Prosthesis Replacement: True or false pull field that will display 1 if the Prothesis checkbox is checked on the Incident tab and the Initial Placement checkbox is unchecked. 0 will be pulled if these conditions are not met.

  • Is Orthodontics: Displays T for true in the note when the Orthodontics box is checked and F for false when it is unchecked. This checkbox is only available in MacPractice DDS.

  • Occupational Illness: True or false pull field that will display 1 if the Occupational Illness/Injury checkbox is checked on the Incident tab and 0 if this box is unchecked.

  • Is Orthodontics: Displays T for true in the note when the Orthodontics box is checked, and F for false in the note when the box is unchecked. In order to see the Orthodontics checkbox on the incident the "Show Orthodontics Checkbox" preference must be enabled under Preferences > Ledger > Incident tab.
