EHR Preconfigured Section - Vitals

The Vitals section is used to enter the patient's vitals. An entry added here will then show in the Vitals widget in the Clinical Summary. Vitals entries that already exist in the Clinical Summary can be pulled into the form manually.

This section CANNOT be used on Clipboard forms (Not a patient-facing section).

This section CANNOT be used on Patient Portal forms.

The contents of this section are as follows:

  1. "Vitals" link: In the resulting popover, enter patient vitals for that visit

    • Incident: Select the incident the vitals record belongs to (Incident the form was created under will pull automatically, but can be changed)

    • Date Measured: Date of record (will populate automatically, but can be changed)

    • Time Measured: Time of record (will populate automatically, but can be changed)

    • Weight (lbs): Weight recorded

    • Length/Height (ft/in): Length/Height recorded

    • Blood Pressure (mmHg): BP Recorded

    • Temperature (ºF): Temp Recorded

    • Heart Rate (BPM): Heart Rate recorded

    • Sp O2 (%): Blood Oxygen percentage level

    • Resp. Rate (Breaths/Min): Respiration rate recorded

    • Inhaled Oxygen Concentration (FiO2) dropdown: method and percentage of oxygen intake. “Room air” is the most common and therefore the default selected option.

    • Head Circumference (cm): Records the head circumference measurement for children (this field only appears for patients 2 years old and under)

    • “Cancel” button: allows the user to exit the Vitals window without adding a record

    • “Done” button: allows the user to commit Vitals data to the patient record and form

    • “Delete button: After a vitals record is added, the user can select that particular record and a “Delete” button will be added. If clicked, only that particular vitals record will be deleted.

  2. Data Pulling popover (⤓ in top-right corner of section): Select to preview and pull from existing vitals records. If this button appears blue, this means there are past vitals records that can be pulled in.

If any vitals data entered falls under abnormal or problematic ranges, the user will get an alert and notes can be entered pertaining to that problem.

Vitals collected in this section help populate Growth Charts that can be accessed from the Vitals widget in Clinical. See this article for more information on Growth Charts.

Entries added here will show in:

  • Vitals widget in the Clinical Summary

  • Continuity of Care document (CCDA) printed from Clinical

  • Reports that pull information for Clinical and Government Programs data
