Common Rejections - Missing/Invalid Diagnosis Code Pointer
Rejection Message:
Emdeon Reject DIAGNOSIS CODE POINTER IS MISSING OR INVALID. MUST POINT TO A VALID DIAGNOSIS CODE.
This rejection message indicates that one or more procedure codes on the claim do not have a pointer to indicate what diagnosis is associated to the procedure. This occurs when one or more procedures on the claim do not have a diagnosis code entered in the charge window of the ledger.
To fix this, go to the patient's ledger and open each procedure that appears on the claim. Make sure that each procedure has at least one diagnosis in the Diagnosis tab of the charge. Once this is done, rebuild the claim in the eClaims ability and send.
